Simple Care Level

Ideal for mild hormone imbalances, minor digestive issues and/or fatigue

Initial consultation: 45 minutes with the provider
Follow-up visits: 20-30 minutes as needed (typically 1-2 follow-ips)
Labs ordered: Basic panels (e.g.,CBC, CMP, thyroid panel, vitamin D)
Supplements/Medications: Standard foundational supplements or minimal prescription support

Intermediate Care Level

For moderate hormonal imbalances, gut dysfunction or metabolic issues

Initial Consultation: 60 Minutes with the Provider
Follow-up visits: 30-45 minutes (2-4 follow-ups typical over 3-6 months)
Labs ordered: DUTCH test, GI Map, advanced thyroid/adrenal testing, food sensitivity panel
Supplements/Medications: Personalized protocols with supplements and potential prescription hormones or GI support

Complex Care Level

For chronic or multi-system conditions - autommunity, long-covid or vax inj

Initial Consultation: 75-90 Minutes with the Provider
Follow-up visits: 45-60 minutes (frequent follow-up over 6-12 months)
Labs ordered: Comprehensive functional labs (e.g., DUTCH, OAT, mycotoxin, heavy metal, mold panels, full GI mapping)
Supplements/Medications: Intensive therapeutic protocols including layered supplement plans and specialty prescription therapies.

All Care Levels include personalized treatment plans and email support

Lab testing and supplements are billed separately and tailored to each patient's needs.
Packages and memberships may be available for on-going care.

WEIGHT LOSS / LONGEVITY Consultation Fees

Initial Consultation: virtual visit, then monthly fee

Includes Basic Lab Panel (ordered prior to visit) and RX, 20 mins
Upgrade to Comprehensive Lab Panel for additional fee
Semaglutide, Tirzepatide, Sermorelin, NAD

PAYMENT AND INSURANCE INFORMATION

We accept all major credit cards, debit cards, and payment through HSA/FSA accounts - you will be billed through QuickBooks at the time of your consultation.
We do not accept insurance, but we provide a receipt with codes for you to submit to your insurance for potential reimbursement. Some insurance companies may reimburse for labs or consultations, but we encourage you to check with your insurance provider prior to scheduling.

CANCELLATIONS and NO SHOW POLICY

We require a 24-hour notice for appointment cancellations.
Late cancellations or missed appointments may incur a $75 fee.
